I, the registered owner above, acknowledge that I am the primary owner responsible for all decisions made for all pets that are registered in my account. I will be the primary contact person with Amber Vet/Amber Cat Vet.

I also acknowledge that my information and my pet's information may be provided to 3rd party vendors when necessary to fufill a requested service for my pet, such as external laboratory tests, prescriptions for compounded medications / prescription diets, etc.
